Registration Information

We want to keep it simple for you, so please following these 5 easy steps

1:    First print the right form for the performer or performers you want to add

2:    Decide who will be the contact person and get them to fill out the Contact Person Form

3:    Supply a photo of the performer or performers, this can "ONLY" be a head and shoulders photo in front of a blank background  

       This can be a print which we will scan /  on CD to copy (These can be returned if you enclose a stamped address envelope)    

4:    Collect Payment of £10.00 per performer by cheque  payable to Mr. Cernik (This is a one of payment and will keep them on the site till they or 16years old)

5:    Once you have all the paper work, photo of performer/s & contact person then send it all (with correct postage please) to
